With the number of people who have careers that involve sitting at a desk for extended periods of time throughout the day comes chronic complaints, like low back pain, neck pain and headaches. The COVID-19 pandemic has definitely contributed to this as well. People are spending more time at home, working for hours straight while not getting out of the same chair more than a couple of times per day. Today we are specifically going to discuss how sitting for extended periods of time can contribute to low back pain.


While sitting in a chair, on a couch, in the car, etc. our hips are in a flexed position. This flexed position can cause the lumbar and SI joints to be fixated and contribute to spasm and contracture in the muscles surrounding those joints. Affected muscles include the lumbar erector spinae and the iliopsoas. A neurologically shortened iliopsoas muscle can contribute to an anterior pelvic tilt, thus increasing the lordosis in the lumbar spine. This can soon cause the lumbar erectors to shorten as well. Pain, whether local or peripheral, can then appear as compressive forces disrupt the function of facet joints, lumbar discs and spinal ligaments.


So if those muscles are tight, what is happening to their antagonist muscles?

Antagonist muscles affected in this lower crossed syndrome are going to be the glutes and the core. Hence, WEAK ABS AND ASS. Core support and stability is lost as the rectus abdominis and transverse abdominis muscles are overpowered by the strong pull of the iliopsoas and erector spinae groups. With this type of a complaint, the goal is to balance out the muscular imbalance described above.


A variety of different conservative care techniques can be beneficial in manually lengthening the spastic and shortened muscles. However, true relief will not be attained unless the appropriate core stability and glute activation exercises are incorporated into a treatment plan.
